1. What you need to complete the Account Transfer Form
- Wachovia Bank, NA ("WBNA") account number into which the funds will be transferred
- Account number(s) from which you are transferring funds
- Name and address of the present custodian or trustee
2. Once you’ve completed the Account Transfer Form
Send a printed copy of the completed form with your recent account statement to:
